View Crisis ResoucesThe Health Records – Release of Information department is responsible for maintaining the confidentiality, accuracy, integrity and quality of patient information for CAMH. These services include processing of requests for patient information.
Patient information provided to health care providers is free of charge; however, fees may apply for other requestors.
Complete and sign the Request for Access to a Record of Personal Health Information form. See the condensed fee schedule below for more information. Please include a picture or photocopy of government issued photo ID when submitting request.
Note: You have the right to access your personal health information at CAMH, unless a legal exception applies under the Personal Health Information Protection Act, 2004. All requests for access to a record of personal health information must be submitted through the Release of Information department through the request form linked above. CAMH has 30 days to respond to your initial request for access.
A health record(s) can only be released to a third party if signed consent is provided by the patient (or the patient’s substitute decision maker), unless the Personal Health Information Protection, Mental Health Act, or other applicable legislation authorizes its disclosure without consent.
CAMH’s Consent for Disclosure of Personal Health Information form (or equivalent consent/authorization) must accompany a request for release of a health record (unless exemption applies). The Consent for Disclosure of Personal Health Information must be dated and witnessed.
Provide proof of legal authority with a written request to the Release of Information department outlining the records required. You must provide either:
- a copy of the deceased person’s will naming you as the executor; or
- in the absence of a will, “Certificate of Appointment of Estate Trustee” signed by a registrar or the court which can be obtained from the Estate Registrar Office, 393 University Avenue, 10th floor
In accordance with CAMH’s retention policy, we retain patient records for 35 years.

Please note: Our services are centralized at the Queen Street location and are responsible for releasing information from the entire hospital.
An initial administrative fee and additional charges may apply. Please see the condensed fee schedule below for common requests. CAMH accepts payment by cash, credit card, debit and cheque. Please make cheques payable to CAMH.
